Insurance Plans


Insurance Companies we work with are listed below:  Please verify coverage and obtain an authorization prior to your first visit. 

You are responsible for obtaining prior authorization for treatment from your insurance carrier.  

 

Please be sure to call and obtain an Authorization Number which you will bring to your first session.

 

Often your employer may have an EAP (Employee Assistance Program) where no co payment is necessary. Do look into this.



As a courtesy to you we will bill your insurance, however, you are responsible for the deductables, and copayment amounts as set forth by your benefit plan and for any uncovered benefit charges. 

Out of network plans such as some Blue Cross Plans must be paid in full at the time of the appointment.  Your plan will reimburse you for the allowed difference.
